An initial session (usually of 1 or 2 hours duration):

  •  This is a person-to-person consultation between the pharmacist and the patient.
  • During this session, the pharmacist collects information on the patient’s medical history, including treatment history (current medications taken by the patient), important lab results, social history, over-the-counter (OTC) medications or supplements, adverse medication reactions, adherence history, and any subjective concerns.
  • The pharmacist spends dedicated time reviewing the information provided by the patient and provides medication education while addressing other concerns as needed

Goal Of this session

  • To identify and prioritize medication-related problems (MRPs) 
  • To suggest any recommendations for therapy optimization

A few days after the session, the patient is provided with 2 documents:

– A follow-up session is decided upon at the end of the CMR session.

– Any new patient should reserve this first session one time as long as it is reserved within a maximum of a year of the first session.
