The pharmacist follows-up on patient outcomes (about 20 minutes duration)
Any follow-up session can be reserved as long as not more than a year has passed since the initial CMR session. If more than a year has passed since the initial CMR, the patient has to register for a new CMR
Session Policy
Follow-up sessions are usually decided upon by the pharmacist.
If the patient desires more follow-up sessions, these can be reserved as follow-up sessions or targeted medication reviews (TMRs) if they concern a single medication or disease state.
A few days after the session, the patient is provided with 2 documents:
- Personal Medication Record (PMR) listing all their medications with instructions on use
- Medication-related Action Plan (MAP) or a To-do list that guides the patient on achieving better outcomes/ target goals with chronic disease management A follow-up session is decided upon at the end of the CMR session Any new patient should reserve this first session one time as long as it is reserved within a maximum of a year of the first session.
